Delete Your Account

We're sorry to see you go. If you'd like to delete your TakeCare account and all associated data, please fill out the form below.

What gets deleted

  • Your account profile and login credentials
  • All health data you've logged (meals, symptoms, sleep, exercise)
  • Your conversation history with Carey
  • Root Cause Explorer analyses and results
  • Any subscription information
This action is permanent and cannot be undone. Please make sure you want to proceed before submitting your request.

Delete from the app instead

You can also delete your account directly from the TakeCare app:

  1. Open the TakeCare app
  2. Tap the profile icon in the top right corner
  3. Scroll to the bottom of your profile
  4. Tap Delete Account and confirm

Submit a deletion request

If you can't access the app, fill out this form and we'll process your request within 30 days.
